2026 Aspen Summer Adventure
Application & Program Information

Program Dates: June 6-11, 2026

Application Process: The following steps must be completed before your child can officially be accepted as a participant for our Aspen Summer Adventure Program:

  1. Click here to complete and submit our Aspen Summer Adventure Health & History Form.

  2. Click here to complete a waiver and medical consent form.

  3. Click here to view the Aspen Summer Adventure Guidelines form. Please read and complete the entire form with your child. Both the parent/guardian AND child must sign this form together.

  4. Click here to upload a a recent photo of your child AND copy of your insurance card(s) (front and back). If you are not able to upload files, please email them to Office@ShiningStarsFoundation.org
